Donor-Derived Neuroendocrine Carcinoma Transmission to Two Kidney Transplant Recipients Demonstrated by Short Tandem Repeat Analysis: A Case Report

Cancer transmission from a donor organ to transplant recipient is rare but not infrequently fatal event. We report case of lung cancer deceased 2 kidney recipients. Approximately 1 year after uneventful transplantation, both recipients developed acute failure. Computed tomography imaging abdomen and pelvis for showed masses in the transplanted kidneys along with innumerable livers. Pathologic examinations cases demonstrated high-grade neuroendocrine carcinoma mirror image histologic findings liver metastases. Short tandem repeat (STR) analyses were performed determine origin tumors. STRs tumors nearly identical that donor, proving same donor. Immunohistochemical positive thyroid transcription factor 1, supporting primary. One died as direct sequela metastatic tumor, other required nephrectomy chemotherapy. Awareness this largely nonpreventable complication prompt molecular testing if suspected are important.
